It just sprang to mind, one night as a nurse I was involved in a case of drug dose for a child. I wouldnt counter sign because I knew the dose wasn’t the right volue of injection for a child. More like ten times the volume. But all the formularies on the ward stated the Dr prescribing was right. The other nurse was well peed off with me. I made the on call pharmacist come to the ward and demonstrated the volume I was conscerned about. He went away and studied all the books she had. Its seemed after hours he came back to the ward and agreed with me, the dose prescribed was never given. The source of the error was the Liverpool paediatric formulary which had a misprint by a factor of ten. All the other formularies replecated this error.
The child would have died and both nurse giving the wrong dose would rot in jail. The Dr prescribing the dose would have received a slap on the wrist. Thats the way it works because the person giving the does is the responsible person.

I agree that the chances of the error in the publication not being noticed and corrected are small (I would have thought the first time would be enough). My old Xeon processor equipped computer suffered a terminal fault this evening, seemingly a defunct processor. That computer was pretty powerful in its day, benchmarking close to 10,000 and in an enormous tower case that was mostly full of numerous empty 5.25 and 3.5in bays - I only used one 2.5in SSD - I've just ordered a refurbished desktop to replace it that is slightly higher on benchmark score but is only about the size of a thick paperback.

Mate just gave me a Sound Devices MixPre-10 that he'd managed to stuff up by leaving batteries in it - very expensive bit of kit...
Got it working again and had a play with it - deeply sexy in a very audio-geeky way! Hugely functional bit of pro kit, built like a tank, with an awful lot packed into a very small space.

Anyone else having trouble getting on to the AoS forum? I’m getting a host server timeout error.
The world looks so different after learning science. For example, trees are made of air, primarily. When they are burned, they go back to air, and in their flaming heat is released the flaming heat of the Sun which was bound in to convert air into tree.

He still owns it, but he hasn’t been active on the forum for a couple of years, apparently due to ill health. The forum itself was still very active up to a couple of days ago, but I guess it’s possible he may have stopped paying the bills.
