Ta for the link BTW, yep cut to any size and shape is well useful. Decades ago used to be a firm called Ramar used to do that, but had to chuck it in cos the postage and packing costs were getting so high. My answers to the original questions:

1. I would always twist heater pairs

2. I would normally reference the heater CT to 0V. If it's not got a real actual CT, fake it with 2 equal resistors.

What about 5K S/E transformers in reverse ? What sort of ratio would that be ? Only trouble with that is the secondary winding is very small, not many turns. Frequency response could be tres naff. At the moment the KT66 SET amp is the only "proper" amp of mine still operational ! Oo-er... Still on the list of things to do is to replace the KT66 with PT15 but it stays as it is till the 300b is back in its rebuilt form. Ah so ... http://livinginthepast-audioweb.co.uk/images/misc/umm_...

You think it's a case where it must have absolutely 12VDC? If so I would say not, must be a regulated 12V I would have thought. Can't you modify the regulator? Sounds a bit like one of those cassette motors that have their own built-in speed regulation. Had a poke about with one many years ago; the ...
