A differential grounded grid might work, cartridge between the two cathodes. DC on the cathode isn't a problem then.

Yep but thats the problem, you have the DC bias on the cartridge, and you start having problems getting the 100R or so that that a cartridge like the 103r needs.

Yep, thats the idea, or turn the heat sink (and fet, so a bit of a pain) round the other way so the fet can be closer to the edge of the board.

If you made the link to the centre pin of the mosfet a top trace, there would be no need to have any traces above the mosfet pins, then the board could stop there.
